Whisk together egg yolks and brown sugar. Over medium-low heat, whisk together the whole milk, heavy cream, cinnamon, and nutmeg.
Stir occasionally until it begins to steam. Slowly pour the warmed milk mixture into the egg yolks, whisking constantly as you pour.
Pour mixture back into the saucepan and return to medium-low heat. Stirring constantly, cook the eggnog just until it thickens slightly and reaches 160°F.
Remove pan from heat and whisk in the vanilla. Serve hot or iced.
